Senior CareScorecard

Methodology and sources

Nursing homes

Nursing home data comes from the Centers for Medicare & Medicaid Services (CMS) Provider Data Catalog: the Provider Information, Health Deficiencies and Penalties datasets at data.cms.gov. CMS refreshes these files monthly. Star ratings (1 to 5) are assigned by CMS. Deficiency counts on list pages come from the most recent standard health inspection cycle. Fines are federal civil money penalties CMS reports for the past three years. "Actual harm" and "immediate jeopardy" refer to citations CMS rates with scope and severity codes G through L.

Assisted living and residential care (Missouri)

The facility list comes from the Missouri Department of Health and Senior Services LTC Directory, published at data.mo.gov. Inspection results come from the department's Show Me Long Term Care site, collected slowly with pauses between requests. Wording is reproduced as published. The memory care list uses the directory's Alzheimer's special care unit flag.

What the scorecard shows

The ratings on Senior Care Scorecard are not our own. Nursing home ratings are Medicare's 1-to-5 star ratings. Assisted living and residential care have no government star rating, so we show license details and state inspection results instead.

Sources for general rules

Answers that explain program rules, rather than facility data, are checked against official sources, last reviewed October 15, 2026:

Limits

Inspection findings describe conditions on the inspection date and may have been corrected since. Government data can lag or contain errors. We do not edit ratings or add our own scores. Always confirm details with the facility and the original source.
