Method
- Homes: all 487 Missouri nursing homes in the CMS Provider Information file (processed Sep 1, 2026).
- Harm-level citation: at least one health citation with a CMS scope and severity code of G through L (actual harm or immediate jeopardy) in the home's three most recent standard inspection cycles, including complaint inspections in those cycles. Immediate jeopardy is J through L.
- Fines: federal civil money penalties in the CMS Penalties file dated from Sep 1, 2023 to Sep 1, 2026. Payment denials are counted separately.
- Areas: St. Louis area is St. Louis City, St. Louis County and St. Charles County; Kansas City area is Jackson, Clay, Platte and Cass counties (Missouri side only); Springfield area is Greene and Christian counties; Columbia area is Boone County.
- Scorecard score: our 0 to 100 score (method). Short-stay hospital units and new homes aren't averaged.
- Limits: CMS data can lag inspections by weeks or months, and homes can dispute citations. A citation is a finding by state inspectors working for CMS, not a court ruling.
